Congratulations for making it this far! As per the terms and conditions of the engagement contract, you are required to fill out the mid-placement feedback form. However, this should be seen as an opportunity to troubleshoot any misaligned expectations between the program participants. After you have completed your second or third Launchpad meeting, please fill out this form.

There are 7 questions and should only take a few minutes to complete. Your responses to the feedback form will only be visible to the SEEnet team. Your feedback is valuable and will be taken seriously. If either the mentor or mentee have flagged the arrangement as non-beneficial then a SEEnet representative may intervene.
